Tools and resources
In partnership with the Office for Public Management (OPM) we have developed tools and resources to support the person centred care agenda under four categories:
- Directory of self-management resources – a one stop shop of 120 online resources for commissioners, patients, clinicians and other providers to support the self-management of health and well being.
- Directory of resources to measure progress in person centred care –menu of tools enabling commissioners, providers and other users to measure clinician / patient participation in shared decision making and self-management.
- Directory of levers to embed person centred commissioning – summary of incentives and levers available to help commissioners influence the uptake of person centred service provision by providers, practitioners and patients.
- Peer learning to commission differently – good practice, case studies and key insights to help commissioners manage long term conditions and reduce the need for emergency care through personal care planning, self management and working with the voluntary and community sector.
