Directory of resources to measure progress in person centred care

A directory of measurement tools to help commissioners, care providers and others demonstrate progress towards the delivery of person centred care.

Demonstrating the success of your person centred care activity is essential to maintaining your programme of work and extending its reach. However, gathering robust evidence of impact and demonstrating how and where successes have been achieved can be a challenge for many programmes.

The following tools have been developed to help measure progress made by clinicians and patients participating in shared decision making and self-management in terms of their beliefs, behaviours, empowerment, communication skills and the quality of the consultation:

  • Menu of Measures – directory of validated measurement tools which includes a description and link to each tool – search for the most relevant tool for your activity using the filter questions
  • Measuring Success – support guide to develop a robust evaluation which includes instructions on how to use the Menu of Measures and guidance on building a programme evaluation plan.

These resources have been identified, reviewed, categorised and developed by Office for Public Management (OPM).
